Body

The corrective and preventive action (CAPA) process is one of the most important elements within a medtech company’s quality management system (QMS). The goal of the CAPA system is to identify, address, and prevent systemic issues that could compromise product safety, regulatory compliance, and the overall operations of the business.

Despite its importance, however, a lot of companies struggle to implement a CAPA management system that’s both effective and efficient. Year after year, deficiencies in the CAPA process are a leading cause of Form 483 observations during U.S. Food and Drug Administration (FDA) inspections.

So, in this article I want to outline the five key components of a strong CAPA system and look at some of the common challenges medtech companies face with CAPA—and how to overcome them.

Pre-send checks for supplier CAPA packs

Useful framing — especially calling out weak RCA and closing CAPAs on “action complete” instead of effectiveness evidence. Those same gaps show up when we review supplier 8D / CAPA packs before they go to a customer.

Practical checks we run before send:

1) Problem statement is quantified (what / where / when / how many) and the suspect population is bounded (lots, date codes, serials) — not a restated complaint.

2) Root cause has two chains with evidence: occurrence (why it was made) and escape (which control should have caught it, and why it didn’t). “Operator error” or “inadequate training” as a final cause almost always gets bounced unless the process/system gap is named.

3) Containment is a verifiable barrier (hold / sort / firewall with quantities and dates), not “operators informed.”

4) Permanent action is a process or detection change you can still see in a year — retraining or 100% inspection alone is usually containment support, not the close.

5) Effectiveness has a defined window and metrics before/after; D7 / system updates (PFMEA, control plan, WI) carry revision IDs and a read-across to similar parts/lines.

Reading the pack once as the customer’s auditor — and asking “what document or measurement proves this claim?” — catches most rejections before they leave the plant.

Disclosure: we built a free advisory pre-send check for supplier 8Ds/CAPAs (no invented evidence): https://8d-gate.vercel.app/?utm_source=qualitydigest&utm_medium=comment&utm_campaign=capa-5-elements
