(IHI: Cambridge, MA) -- The Institute for Healthcare Improvement (IHI) has announced details of its 13th Annual International Summit on Improving Patient Care in the Office Practice & the Community. The popular event brings together health care professionals working in primary care and community settings who are striving to make critical connections across care settings, while containing costs and providing the best patient experience. The conference will take place March 18–20, 2012, in Washington, D.C.

This year’s International Summit will focus on “rediscovering conversations” that are at the heart of transformational care, and will feature:
• More than 50 workshop sessions around key topics, including: new models of primary care, patient- and family-centered care, health IT, coordinating care, and accountable care organizations (ACOs)
• Two keynote presentations and five special-interest keynotes
• 12 half-day mini-courses that offer plenty of hands-on learning
