I was wondering how you are supposed to deal with an out of spec condition in an X-bar&R chart. You can have a sample be out of spec, but have the subgroup average be fine. I have tried with different data and it is very easy to get the X-Bar&R values both to show up in control on your chart even with an out of spec sample.
