I have two questions regarding the FMEA. First, I have read that "feeding" the FMEA by either the process map or the most highly ranked variables from the C&E Matrix is acceptable. My question is which is the preferred way and why. Second, I have also seen FMEAs that calculate the RPN by Severity*Occurrence*Detection and those that exclude detection, i.e. Severity*Occurrence. Is this legitimate and if so when is it appropriate to leave out detection? Thanks.
